Individuals seek healthcare by visiting their primary care providers or consulting with specialists as their primary care providers recommend. Population-focused nursing may soon alter this way of healthcare by preventing and treating healthcare-related issues per individual. Population-focused nursing is defined as evaluating the healthcare needs of a population and making healthcare decisions for the people rather than the individual. Each population is based on everyday environments and common personality traits. Healthcare professionals use population-focused nursing when they use treatment guidelines for certain people. Community assessments are completed to make up the guidelines that identify the issues of health concern that each community is and will be exposed to. A community assessment's purpose is a process in which individuals become better acquainted with the community and populations by reviewing factors that affect the population's health. Healthcare providers such as nurses conduct community assessments to identify strengths, weaknesses, gaps, and risks. Providers get insight and user input used to work with populations from each community assessment. Students can establish themselves with successful interventions collaboratively by using community assessments. When students complete these community assessments, they positively contribute to the community and the at-risk populations with positive changes. The community assessment of Sentinel City was conducted using a demographics assessment, neighborhood safety assessment, community assessment windshield survey, and population health scavenger hunt.
