INTRODUCTION
Nursing is defined as the process of healing provided through a systematic review of patient’s suffering and feelings, evaluated by the best evidence. (American Nurses Association, 2017) A nursing process is a holistic approach to care for the patients considering their physical, mental as well as social development in order to provide an independent, healthy and content living. (Doenges and Moorhouse, 2012) The given case study is based on the nursing process provided to a patient who had a septoplasty and ethmoidectomy surgeries done. Septoplasty is a surgical process to rectify the displacement error of the septum that divides the nostrils of the nose. (Mayo Clinic, 2017) The ethmoidectomy refers to the voiding of the infected tissues or bones in the ethmoid sinus that blocks the nasal track. (Icahn School of Medicine at Mount Sinai, 2017)
The essay describes her pre and post-operative conditions, the nursing care provided postoperatively using a complete nursing care plan and guidelines. Furthermore, the postoperative education required for the patient including risks and potential complications are also mentioned, in addition to the involvement of the interdisciplinary team involved in this case.
BACKGROUND OF THE CONDITION
The ISOBAR tool format is used for detailed patient information and for clinical handover, carried out by the means of observation and communication. (Clarke, 2014)
My name is XXX and I am a student nurse. I am looking after Kelly Malone who is a 49 years old female. Kelly complains that she has been experiencing shortness in breathe through her nose. Due to this, she has to mouth breathe, to receive enough air and avoid fatigue and exhaustion. She states that due to this she has a history of disrupted sleep pattern. She is now assessed by an ENT specialist surgeon who advised Kelly to undergo a surgery for septoplasty and a right ethmoidectomy. Her preoperative observations are T-36.4°C, HR-69bpm, RR-18pm, BP-119/70mmHg and O2-94% RA. Postoperatively the observations are T-36.2°C, HR-68bpm, RR-18pm, BP-111/73mmHg and O2-93% RA. Post surgery she is awake and alert at 11:30hrs. She has a nasal bolster under her nose and has a moderate flow of sanguineous ooze. She has been prescribed with Paracetamol 1g IV/PO 6 hourly, Celecoxib 200mg PO BD and Tramadol SR 100mg PO BD. The recommendations required...
