1 Describe your care management experience in a community based or inhome setting: I work in a critical unit in a hospital setting but I would love the opportunity to expand my skills outside of a hospital setting.
2 Describe your most professionally challenging casepatient how did you work with the casepatient: I had a patient several months ago that was dying she had cancer and developed DIC and there was nothing the doctors could do to stop the bleeding and we had to give her blood. The physician went in to talk to her about going home with hospice and she didn’t want to hear it or talk to the doctor and told her to get out of the room. I asked the patient if I could speak with her one and one alone and she agreed. I asked her what she wanted no one else but her and she started to cry and told me that all she wanted to do was go home and spend her remaining days with her grandkids but her daughter did not want her to she wanted her to stay in the hospital. After listening to her wishes I said if I could make it all happen would she except the hospice care and she said yes. So I called the case manager to get it all arranged and the patient got her wish and she cried when they came to pick her up and take her home and she said I was her angel.
3 How would you engage a member to accept Humana At Home services: I would listen very closely to there wishes and explain why it would be in their best interest to except the services that would help them the most
you help them manage their care: I would set them down and teach them the disease process and what impact it could have on their health not to follow the proper diet and medication regimen and how important it is to keep all follow up appointments and how they need to be 100 percent involved in their own care and teach them some independence on keeping up with a diary about their help and documenting it daily
5 What is the greatest number caseload of patients you have managed in the past: Well like I said I have only worked in a hospital setting and done prn work in a nursing home but I have had lots of patients over the years
6 What is the greatest number of weekly home visits you have managed in the past: I have only worked in a hospital setting but I help my elderly father manage his care at home
supervisor and other support via phoneemail: I am very good at working independently I am a good self motivator and I love taking charge of my own responsibilities when it comes to caring for others
8 What is your comfort level working with computer based systems: Very comfortable as we use them at my job
