Ek die ondergetekende,
Jan Pensioenaris,
verklaar hiermee onder eed in Afrikaans.

Ek is ‘n volwasse afgetrede man wat gewerk het as stoker vir die treine (SASenH), 65 jaar van ouderdom, woonagtig te 12 Smit Straat Potchefstroom 2520. My identiteitsnommer is 5111126064080 en kontaknommer (018)123457.

Op 30 April 2015 het ek my pensioen ontvang wat ek wil belê en sodoende van die rente wat ek op die eenmalige bedrag verdien gebruik om my uitgawes te dek. My goeie vriend Jan Botha(oorlede) het die beleggingsfirma Piet Belegging Edms Bpk voorgestel om die belegging te hanteer. Hy het al by die adviseurs belê en is dus vertroud met hulle werksaamhede. Die finansiële adviseur met wie ek in kontak gekom het is Piet Swart.

Ek het ‘n afspraak vir die 3de Mei ongeveer 12:00 gehad by die kantore van die besigheid te 12 Olivier Straat, Potchefstroom. Tydens die vergadering was die teenwoordige persone ek, Piet Swart en nog ‘n man met die naam Gert Viljoen.

Swart het vir my gesê dat die opbrengs op die belegging sal 10% per jaar wees, maandeliks uitbetaalbaar. Hy het aan my verduidelik dat ek die eerste keer rente verdien binne 3 maande wat ek belê het, ek was egter nie bekommerd nie, omdat Botha so vertroud was met die beleggingsfirma. Swart het veder aan my verduidelik dat ek R200 000 sal belê, waar R100 000 aan Smit gaan en R100 000 aan Viljoen, omdat dit so unieke geleentheid is dat ek nie met een bedrag van R200 000 kan werk nie, maar 2 van R100 000 egter volgens voorwaardes korrek sal wees.

Ek het R100 000 aan Swart bemaak, per tjek met die rekening nommer: 1234 (Bylae DS01). Die ander tjek van R100 000 het ek aan Viljoen bemaak, met rekening nommer: 1235 (Bylae DS02). Ek is ‘n rekeninghouer van BLABSA bank met rekening nr.4637467382083 (Bylae DS03). Ek het my persoonlike besonderhede aan hulle gegee wat hulle op ‘n vorm voltooi het en hulle het my bankbesonderhede geneem om die rente aan my terug te betaal, ek het ongelukkig geen afskrifte van hierdie dokumente nie.
